Home Health & Hospice The Home Health and Hospice Clinical Manager assists the Director of Clinical Services/Administrator with the supervision of home health and hospice services to patients in their place of residence in accordance with physician orders and strives to provide the highest quality of care. Skilled nursing and other therapeutic services furnished are under the supervision of the Home Health and Hospice Clinical Manager. Responsible as a member of the interdisciplinary and clinical management teams to the DCS/Administrator for the implementation, delivery, coordination, and supervision of Hospice services delivered by respective team consistent with Hospice policies and procedures.The Home Health and Hospice Clinical Manager is available during operating hours (or appoints a similarly qualified alternate) and participates in all activities relevant to professional services furnished, including the development of qualifications and assignment of personnel.QUALIFICATIONS: 1. Graduate of an approved school of professional nursing or therapy school and currently licensed in the state(s) in which practicing.

2. Three to five (3-5) years of experience, preferred. 3. One (1) year experience in home health, preferred.

4. Acceptance of philosophy and goals of Agency. 5. Ability to exercise initiative and independent judgment.

6. Ability to work with individuals to enlist cooperation of many people to perform/achieve a common goal. 7. Ability to respond appropriately to crisis outside of a hospital setting.

Acceptance of and adaptability to different social, racial, cultural and religious modes. MENTAL REQUIREMENTS: Must be able to work independently, make judgments based on assessments and data available and act accordingly. Must be flexible, innovative and possess good interpersonal skills. Must be able to cope with mental and emotional stress and demonstrate emotional stability.

**RESPONSIBILITIES**:
1. Understands and adheres to established Agency policies and procedures. 2. Participates in the development of standards which ensure safe and therapeutically effective service to patient/family.

3. Ability to supervise, guide and develop nursing skills and performances. 4. Consults with the Director of Clinical Services/Administrator in determining a staffing pattern which will accomplish stated objectives and promote a maximum level of utilization of health personnel.

5. Assists in recruiting, hiring, evaluating and terminating nursing personnel. Assists in instituting, interpreting and documenting disciplinary action. 6.

Responsible for the orientation of new patient care personnel. 7. Assists in the planning and consultative needs of staff. 8.

Assists in the preparation and implementation of policies and procedures which meet Medicare, Medicaid, accrediting bodies, state and local laws. 9. Displays a willingness to support policies and procedures and uses appropriate channels for changes of such policies. 10.

Assists in the coordination of interdisciplinary team services, patient care and referrals. 11. Conduct appropriate supervisory visits which allow for clinical observation of performance of assigned team members. 12.

Provides Hospice nursing services, treatments and preventive procedures. 13. Teaches, supervises and counsels the Hospice patient and family members about providing care for the patient. 14.

Supervise the planning, coordination and delivery of the interdisciplinary group interventions. 15. Assists in the organization of nursing services to delineate and delegate functional responsibility, lines of relationship and communication to provide safe and therapeutically effective service. 16.

Participates in the coordination of Agency’s services with the services of other community agencies. 17. Participates in studies and research and other administrative functions as assigned. 18.

Serves as a role model for all colleagues by setting an example of high standards in dress, conduct, cooperation and job performances. 19. Observes confidentiality and safeguards all patient related information. 20.

Accepts responsibility for regular attendance and punctuality; fulfills job related requirements without regard to time involved. 21. Serves as a resource person to employees. 22.

Develops a cooperative relationship and communicates effectively and professionally with physicians. 23. Investigates and reports any problem relating to patient care and/or employee well-being. 24.

Immediately reports any accident, incident, lost articles or unusual occurrence to the Director of Clinical Services/Administrator. 25. Attends pertinent continuing education programs other than routine inservices and shares information with staff. 26.

Assists with the Agency’s ongoing Quality Assessment and Performance Improvement (QAPI) program. 27. Assures the development, implementation and updates of the individualized patient plans of care. 28.

Reviews patient medical records for compliance with
