LPAH provides nursing, physical/occupational/speech therapy, and certified nurse’s aide care to patients so they may recover in the comfort of their home. Home health and hospice services supports our vision of enriching lives by providing the right services, at the right time, in the right setting. The agency covers the South Hampton Roads area including the Lake Prince Woods campus, a continuing care retirement community.: The Administrator of Home Health and Hospice Services (Administrator) is appointed and directed by the governing board and reports to the Executive Director.
The administrator is responsible for the day to day direction, coordination and general supervision of home health, home care and hospice programs. Responsibility spans development, organization, implementation and evaluation of agency programs in accordance with the Medicare Conditions of Participation, CHAP Standards of Excellence, State licensure requirements, and the policies and procedures of the agency. The position is responsible for operations, budgets, and oversight of full time, part time and PRN staff including nurses, social workers, physical therapists, occupational therapists, speech therapists, aides, administrative and clerical staff.ESSENTIAL DUTIES AND RESPONSIBILITIES: 1. Planning, organizing, directing and evaluating the operations to ensure the provision of adequate and appropriate care and services.
2. Assuring organizational compliance with legal, regulatory and accreditation requirements. Maintains appropriate licenses, certifications and accreditation. 3.
Along with his /her supervisor, developing and managing budgets of operations in accordance with established fiscal parameters. Monitoring business operations to ensure financial stability, accurate billing and cash collection. 4. Implementing governing body directives and ensuring that appropriate service policies are developed and implemented to accomplish identified outcomes.
Assuring appropriate data collection and regular, complete reports are received by the governing body. 5. Recruiting, employing and retaining qualified personnel to maintain appropriate staffing levels. Evaluating services and personnel using measurable outcomes and objectives.
Assuring the development and qualifications for professional services and the assignment of personnel. 6. Establishing and maintaining effective channels of communication including integration of technology, as applicable. 7.
Ensuring program personnel have current clinical information and current practices. Insuring supportive services are available to staff. Informing the governing body, staff and professional advisory group of current organizational, community, and industry trends. Insuring adequate space, equipment and supplies are available.
8. Directing and monitoring organizational Quality Assessment and Performance Improvement activities. Assuring actionable objectives are derived from evaluation of services and personnel. Monitoring operational progress toward accomplishing operational and strategic goals.
Supports efforts toward excellent patient and family satisfaction. Facilitating conflict and complaint management / resolution. Leading the Professional Advisory Committee for home health. 9.
Assuring staff development including orientation, in-service education, continuing education, evaluation of staff, competency testing and performance improvement. 10.Serving as clinical team leader for home health or hospice. Assuring that skilled nursing and other therapeutic services furnished are under the supervision and direction of a physician or a registered nurse. 11.
Insuring adequate and appropriate staffing. Assuring appropriate staff supervision during all operating hours. Appointing a similarly qualified alternate to be available at all times during operating hours and in emergency situations, in the absence of the administrator. Manages and includes self in the on call rotation.
12.Along with his/her supervisor, assuring the accuracy of public information materials and activities. 13. Leading and promoting the interdisciplinary aspect of hospice care. Directing staff in performance of their duties including admission, discharge, transfer, revocation and provision of service to patients.
Insuring that structure and systems promote interdisciplinary care. Managing agency contracts. Insuring collaboration with agencies and vendors for effective management of services. Insuring services provided by other agencies are authorized by hospice.
Monitoring service utilization to ensure delivery of comprehensive care. 14.Assuring coordination with other organization areas and senior management, as appropriate, according to the structure and services. 15.Assuring standards of
